Proof of Sudbury Residency Requirements

  • Proof of Residency must be completed before a student is allowed to begin school.

    The following three documents are required as proof of being a Sudbury resident.  Please note that post office box addresses will not be accepted.

    Note: If you are living in a home belonging to an extended family member or friend and do not have a formal rental agreement, you will need Evidence of Residency and Evidence of Occupancy in the homeowner's name, parent/guardian Evidence of Identification, and a notarized Residency Affidavit - Landlord/Shared Tenancies document. As with leases, you will need to provide a new affidavit annually. There is a public notary at the school department at 40 Fairbank Road who is available to assist with this form. 

    Homeowners: 

    1. Evidence of Residency: Record of most recent property tax bill (not car excise tax), mortgage statement, deed, or signed and fully executed Purchase & Sale (P&S) Agreement along with signed statement by a Bank Officer or licensed Real Estate Broker certifying that financial commitment has been approved.
    2. Evidence of Occupancy: A utility bill issued within the last 60 days (fuel, electric or water bill).  In the case of families who have not yet moved into the property at the time of registration, a bill may be supplied within 30 days from the student's entrance date.  Should a bill not be provided, students may be required to withdraw from school.
    3. Evidence of Identification: A photo ID of parent/guardian (valid driver's license, valid passport, or other government issued ID).

    Renters:

    1. Evidence of Residency:  A lease and/or Rental Agreement signed by both the landlord and tenant, indicating the names of the tenants, and the effective dates of the agreement or, if a lease or rental agreement is not available, then a legal, notarized affidavit from the landlord/property owner affirming tenancy and address where the student resides the majority of the time will be required.  Download the Residency Affidavit - Landlord/Shared Tenancies document. Along with the Residency Affidavit, evidence of ownership from the property owner (see number 1 from Homeowner above) is required. As with leases, you will need to provide a new affidavit annually.  
    2.     When a lease expires, a new lease must be shared at the front office of each elementary school or the Student Services office at Curtis Middle School, whichever is applicable.
