New to Sudbury Orientation
There will be an orientation event for families with children grades K-8 at each school on August 24th.
If you have registered new students in grades 1-8 for the fall of 2023 or registered too late for Kindergarten Orientation in May, you will be contacted by your child's school with more information for the August orientation.

Please come in person with hard copies of the Part Two documents listed below to the Student Services Office, 40 Fairbank Road, Sudbury, MA 01776. Please come inside to the office and meet with a staff member to go over the documents. We will no longer accept documents through mail or email.
Please know that your child will not be officially registered until this paperwork is complete.

Below you will find the forms and documents required for each student enrolling in Sudbury Public Schools. Note that some links will download a form to be printed, filled out and dropped off at Student Services, 40 Fairbank Road, Sudbury, MA 01776.

- Please use this checklist. Registration will be incomplete and put on hold if anything is missing.
- This form is also translated into Spanish/español, Portuguese/Português, Russian/Русский, Simplified Chinese/简体中文, and Haitian Creole/Kreyòl ayisyen.
Forms to Fill Out:
- Health Screening Form
- Print and complete a copy for each student.
- This form is also translated into Spanish/español, Portuguese/Português, Russian/Русский, Simplified Chinese/简体中文, and Ukrainian/українська.
- Parent Information Form
- Print and complete a copy for each student.
- This form is also translated into Spanish/español, Portuguese/Português, Russian/Русский, and Simplified Chinese/简体中文.
- Request to Transfer Records - Release of Information
- Military Form (if applicable)
- Preschool Inventory Form
- For Kindergarten registrations only.
- This form is to be filled out by the current preschool teacher and mailed to Early Childhood Office at 40 Fairbank Road, Sudbury, MA 01776
Documents to Collect:
- Copy of your child's birth certificate, adoption decree, or US passport
- Copy of most recent doctor's physical form (within the last 13 months), including a lead test*, and immunization records (birth to current date)
- These forms are generated from your child's pediatrician. If needed, you can use the form linked here.
- *Any previous lead test is fine; documentation must show the date of the test and the lead value. You do not need a new test done at the 5-year-old physical.
- Special Education Records - IEP or 504 Plan (if applicable)
- Proof of Residency (POR)
